Woman Arrested in Elmhurst for Allegedly Forging Prescription Drug Forms

Police also report arrests for domestic battery, possession of cannabis and ammunition, and unlawful use of a handicap placard this week.

Jennifer V. Mladucky, 26, 7932 W. 82nd Place, Bridgeview, was arrested on the 100 block of South Cottage Hill at 2:58 p.m. Feb. 19 after police received information about a medical assistant who was forging prescriptions. Police said they searched her car and found several items of drug paraphernalia. She was charged with two counts of forgery and taken to DuPage County Jail. Michael Harris, 46, 531 N. Van Auken, was arrested for domestic battery after an incident at his home at 9:29 p.m. Feb. 20. He was taken to Elmhurst Memorial Hospital, and police said they would charge him upon his release. Elmhurst Police Report Forgery, Identity Theft, Graffiti and License Violations

Forgery Police believe they know who last month stole $525 from Lover Cigar, 176 N. York Road. A store employee checked the account balances and found two withdrawals, for $250 and $275, that he did not make. The bank had surveillance of the woman cashing the checks, and she matches a description of the girlfriend of one of the store's regular customers. The woman was in the store on April 23 and used the restroom at the rear of the store. The victim filed the report on May 3. An investigation is pending. Identity Theft A resident of the 700 block of North Adele Street reported that she was contacted by a representative of DTE Energy Co., based in Michigan, who told her she was late on payments. Fleeing Suspect Creates Commotion in South Elmhurst Monday

Police Chase Forgery Suspect Cornelius Gates, 31, 5442 W. Cortland Apt. 2, Chicago, was arrested and charged with felony forgery after he reportedly attempted to cash a forged check at 1:10 p.m. Dec. 12 at Bank of America, 1000 S. York Road. Gates ran from officers and entered a vehicle that was occupied by Tiffany Sisson, 30, 5439 W. Iowa, Chicago, Jasmine Burn, 20, 155 N. Leamington, Chicago, and another person. Sisson and Burn also were charged with felony forgery, and Sisson was charged with possession of cannabis and possession of drug paraphernalia. All three were taken to DuPage County Jail. Disorderly Conduct Ciro Calderon-Rodrig, 47, 126 N. May Apt. D, Addison, was charged under city ordinance with disorderly conduct at 1:24 p.m. …

Former CFO of Superior Ambulance Pleads Guilty to Theft, Forgery

Woman who made $200,000 salary admits to stealing $42,000.

A Wayne woman arrested a year ago for stealing from her Elmhurst employer pleaded guilty Wednesday to forgery and theft charges, according to the Chicago Sun-Times. Teresa Wolande (who also goes by the last name of Pahl), 34W943 Army Trail Road, pleaded guilty to stealing more than $42,000 from her former employer, Superior Air-Ground Ambulance Service in Elmhurst, between May 1, 2009, and July 15, 2010. Her indictment handed down last year said that while Wolande was employed as chief financial officer at Superior, she wrote checks from the company's account to pay for vacations, car repairs and taxes. As CFO, she pulled in a salary of $200,000 a year. Her home is valued at nearly $5 million, according to the Sun-Times article. The …

Cigarette Causes Deck Fire on South Spring Road

A fire broke out on the wooden deck of a second-floor apartment on the 500 block of South Spring Road at 11:15 a.m. Oct. 19. The resident said he had been smoking a Camel cigarette and extinguished it outside the back door of his apartment into a partially wet cardboard box. He said he thought it had been fully extinguished, but a short time later, a portion of the deck was burning. Theft Someone took a black leather wallet containing a drivers license, bus pass, insurance card and debit card from the lobby of an apartment complex on the 200 block of South West Avenue at 11:30 a.m. Oct. 13. Battery Arrest Melissa Barnes, 30, 3902 Jackson, Bellwood, was arrested and charged with battery after an incident at Citgo, 218 E. Butterfield, at 6:34 p.m. July 22. Barnes allegedly poked and slapped a man. She was also served a no-trespass order for Citgo. She was released on bond.  Battery A male victim reported to police that he was beaten by three young men and a young woman while riding his bicycle home at 1:50 a.m. July 24. He said the suspects kept driving their vehicle in front of him and provoking him. The victim said he got off of his bicycle because he thought it was his friends playing a joke on him. Former CFO Accused of Stealing More Than $42,000 from Elmhurst Employer

12-Count indictment handed down on Wayne woman.

DuPage County State's Attorney Joe Birkett announced today (Sept. 30) that Teresa Wolande, 51, of 34W943 Army Trail Road, Wayne, was indicted on charges she allegedly stole more than $42,000 from her former employer, Superior Air-Ground Ambulance Service, 395 W. Lake St., Elmhurst, between May 1, 2009, and July 15. It is alleged in the 12-count indictment that while employed as chief financial officer at Superior, Wolande (also known as Pahl) wrote checks from the company's account to pay her own personal expenses, including vacations, car repairs and her taxes.  The indictment also alleges that Wolande used company credit cards to make unauthorized personal purchases.  The indictment alleges forgery and obstruction of justice against …
